🎈 Airbag จะทำงานเมื่อการชนรุนแรงมากเท่านั้น และต้องใช้ร่วมกับ Seat Belt เสมอ การบาดเจ็บที่พบบ่อยที่สุดคือ

1. หัว หน้า และ คอ 40% safety Belt จะรัด 3 จุดที่ไหล่ และเอวทั้งสองข้าง หน้าอกจะถูกยึดตรึง แต่ศีรษะกับคอจะยังขยับได้ แรงเหวี่ยงจะทำให้ส่วนหัวกระแทกกับ Airbag จึงพบบ่อยที่สุด

2. แขน 30% เพราะแขนเป็นส่วนที่ขยับได้ อาจจับพวงมาลัยอยู่ในขณะที่ Airbag ทำงาน จึงพบการบาดเจ็บที่ไหปลาร้า แขนหักได้

3. หน้าอก 10% เป็นส่วนที่ถูกดึงรั้งโดย Safety Belt

🤕 การบาดเจ็บ

🧠 ศีรษะ – Fracture Skull และ Intracranial Hemorrhage

👨🏻 ใบหน้า – Nasal Fracture, Maxillary Fracture, Mandible Fracture, Temporomandibular Joint Injury, ZMC (Zygomaticomaxillary) Fracture, Submandibular Gland Rupture

👁 ตา – Corneal Abrasion, Hyphema, Lens Dislocation/Subluxation, Chemical Keratitis, Orbital Fracture, Retinal/Vitreous Hemorrhage, Retinal Tear Detachment, Macular Hole Rupture, Choroidal Rupture

👤 คอ – Cervical Spine Fracture, Internal Carotid Dissection (การวินิจฉัยทำได้ยากเพราะอาการอาจไม่ชัดเจน)

👕 อกและท้อง – Rib Fracture, Bilateral Pneumothorax, Aortic Transection, Heart Valve Injury, Cardiac Rupture, Abdominal Injury, Thoraco-Lumbar Spine Injury (ท้องและสะโพกจะถูกดึงรั้งกับกระดูกสันหลัง เกิด Flexion-Distraction Spine Injury)

💪🏻 แขน – Clavicle Fracture, Radius & Ulna Fracture

🩸 อื่นๆ – Acoustic Damage, PROM in Pregnancy, Burn

👶🏻 ในเด็ก เนื่องจากหัวใหญ่ แต่ตัวเล็ก มีโอกาสเกิดการบาดเจ็บที่หัวและคอสูง มี Atlanto-Occipital Dislocation, C2 Vertebral Body Dislocation, Basilar Skull Fracture

ดังนั้น เมื่อผู้บาดเจ็บมาที่โรงพยาบาล จะต้องระวังเรื่อง

1. Head Injury – ต้อง CT Brain และ Observe Neuro Sign โดยเฉพาะถ้ามีประวัติกินเหล้า

2. Facial Injury – ตรวจ ตา หู จมูก ปากให้ละเอียด Film อาจจะไม่พอ ถ้าแย่มากก็ควร CT Facial Bone ระวังตาเป็นพิเศษเพราะตาบอดได้

3. คอ – ควรใส่ Collar + C Spine Protection เพราะตาม Mechanism of Injury จะมี Whiplash Injury คอสะบัดไปตามแรงรถ (Rapid Forward & Backward Acceleration & Deceleration) แม้ผู้บาดเจ็บจะไม่บ่นเจ็บคอ ไม่มี Neuro Deficit แต่อาจมีการบาดเจ็บที่บดบังอื่นๆ (Distracting Injury) จึงนอกจากนี้ ต้องคิดถึง Internal Carotid Dissection ด้วย

4. อก ท้อง – ต้องทำ eFAST CXR Pelvis Film L-S Spine ถ้าเจ็บหลายตำแหน่งมาก และอาการยังคงที่ อาจพิจารณา CT Trauma Whole Body เพื่อ detect occult injury ด้วย

5. แขน ขา – Long Bone Fracture และ Extremity Bleeding เสียเลือดได้มาก ควร Stop Bleed + Detect Fracture ให้ครบถ้วนใน Secondary Survey
